2. Your device make and model
3. Cause and date of loss or damage
Service fee or deductible payment method options include: credit card, debit card, eCheck or C.O.D. with $17 charge. Under certain circumstances, additional documentation — such as a police report or proof of loss — may be required in order to process your claim. Claims must be filed within the time limit indicated in your coverage documents.
