Customer Proprietary Network Information ("CPNI")

Effective June 1, 2015

At U.S. Cellular®, we're dedicated to providing superior customer satisfaction and our associates and agents are committed to protecting the privacy of your personal information. Earning and maintaining your trust is extremely important to us, as we value your business.

The purpose of this notice is to explain what Customer Proprietary Network Information ("CPNI") is and inform you about how we at U.S. Cellular use such information to provide you with telecommunication products and services that best meet your needs.

While we extensively train our associates to ensure they uphold the protection of your personal information, you have the right to the protection of your confidential information and it is our responsibility to protect that information.

What is Customer Proprietary Network Information?

It refers to and includes information that we have about you by virtue of our relationship with you as your wireless service provider. This includes information about your account usage—such as the type and quantity of the services you subscribe to, and other details about your calling, text messaging, or picture messaging plan. Call detail information includes the date, time, call-to location, number called or the number the call was received from, and minutes and charges of the call. Text or picture messaging detail includes the number the message was sent to or received from, date, and time the message was sent from your phone. Text or picture messaging detail does not include the actual message content. Use of CPNI is subject to federal laws and regulations with which U.S. Cellular is required to comply.

How does U.S. Cellular use this information?

We use CPNI to help us service your account when you call us with questions, to guard against fraudulent use of our network, to provision our services, or to generate and collect your bill. We may also use it to recommend other wireless products and services to you. For example, if you routinely exceed the minutes in your calling plan, we are able to use your calling records to recommend a calling plan that may be better suited to meet your needs. The CPNI rules recognize the value of our use of this information to serve you and permit our use of your information provided we safeguard it from unauthorized disclosure. We want to assure all of our customers that we have practices and procedures in place to keep your personal information safe.

How is U.S. Cellular restricted from using my CPNI today?

U.S. Cellular is prohibited from allowing our agents and the U.S. Cellular family of companies ("Affiliates") from using this information for marketing U.S. Cellular's or its Affiliates' products and services to you, but only if you object. Allowing our agents and Affiliates to use this information will make it easier for them to offer suggestions about new wireless products and services to help you get more out of your existing wireless service, or to assist you with your other telecommunication needs. Because any agent or Affiliate that uses your CPNI works for us they will have the same duty to safeguard your CPNI as we do.

Will you allow anyone else access to my CPNI?

We will never disclose your information to unaffiliated third parties not working for us without your prior consent except to initiate, render, bill, and collect for services provided by U.S. Cellular. However, there may be instances when U.S. Cellular may access or disclose CPNI without providing you a choice in order to: (i) protect or defend the legal rights or property of U.S. Cellular, our Affiliates or their employees, and agents (including enforcement of our agreements); (ii) protect the safety and security of customers or members of the public including acting in urgent circumstances; (iii) protect against fraud or for risk management purposes; or (iv) comply with the law or legal process such as a subpoena.

If I want to object to your agents and Affiliates using my CPNI, how do I contact you?

If you would prefer that our agents and Affiliates cease using your CPNI for the purposes for which permission is required, then you may "opt out"–that is, you may instruct us not to permit our agents and Affiliates to use your CPNI for the limited purposes outlined in this notice. That is your right and it will have no effect on our existing relationship or the services that you currently receive from us. However, it may make it more difficult for us to communicate with you through our agents and Affiliates about new offers and features. Any decision to "opt-out" will remain valid until you affirmatively revoke it.

If you wish to opt-out, simply contact us at any time by calling our toll-free CPNI Opt-out Line (800-509-6254) and follow the recorded instructions. TTY users can opt-out by first dialing a telecommunications relay service (TRS) center, via 711, in order to contact a TRS Communications Assistant (CA). Then, simply ask the CA to dial 800-509-6254 and follow the recorded instructions. Unless you notify us, we may share your CPNI with our agents and Affiliates as described above beginning 30 days after the first time we provide you with notice in your bill or in a text message. Your choice will remain valid until you notify us that you wish to change your election.

We value you as our customer and appreciate the opportunity to serve as your wireless carrier. For additional information regarding our policies and practices for protecting customer privacy, please visit
