Things We Want You To Know
Either a new agreement with a 2-yr. initial term (subject to a pro-rated $150 early termination fee for basic phones, modems and hotspot devices and a $350 early termination fee for smartphones and tablets) or Retail Installment Contract required. Credit approval required for all new lines. The Customer Service Agreement Terms and Conditions apply for as long as you are a customer. A $40 activation fee may apply. Regulatory Cost Recovery Fee of $1.82 applies; this is not a tax or government required charge. Additional fees, taxes, terms, conditions and coverage areas apply and may vary by plan, service, and phone. Use of service constitutes acceptance of the terms of our Customer Service Agreement. U.S. Cellular reserves the right to terminate your service if more than 50% of your overall minutes in each of three consecutive billing cycles are used in U.S. Cellular's non-licensed markets. U.S. Cellular reserves the right to terminate your service if more than 400 MB of your data usage in any month is used in U.S. Cellular's non-licensed markets.

Kansas Customers: In areas in which U.S. Cellular receives support from the Federal Universal Service Fund, all reasonable requests for service must be met. Unresolved questions concerning services availability can be directed to the Kansas Corporation Commission Office of Public Affairs and Consumer Protection at 1-800-662-0027.
Retail Installment Pricing: You may return the wireless device to us in good condition and accompanied with original packaging, battery, accessories and instruction manuals within fifteen (15) days of purchase. If you return the device, you must have valid proof of purchase and will be entitled to a full refund of the purchase price, less a restocking fee of $35.00. After completing the return, and payment of the restocking fee, we will credit the amount of your refund to your obligation to pay the Amount Financed. Promise to Pay: You must enter a Retail Installment Contract ("Contract") and pay us the Amount Financed under the Contract by making the payments in the amounts described in the Payment Schedule. You may prepay all of the unpaid balance of the Amount Financed at any time without penalty. If you want to prepay the unpaid balance, you must contact us and then we will bill you for the full amount owed on your next statement. You will be in default if you: (a) fail to make any payment when required, (b) violate any other provision of the Contract, (c) become the subject of any bankruptcy or insolvency proceeding, or (d) die. If you are in default, to the extent permitted by applicable law, we may require you to pay immediately the entire unpaid Amount Financed as well as our collection costs, attorneys' fees and court costs related to enforcing your obligations under the Contract. Insurance/Extended Warranty: You are not required to purchase property insurance protecting against the loss or theft of the device or an extended warranty contract ("Protection Products") as a condition to receiving credit. You may obtain property insurance and extended warranty coverage from anyone you want. Wireless Service: You are required to obtain voice, messaging or data wireless service ("Wireless Service") from us or our affiliates at the time you enter into Contract in order to obtain retail installment pricing under this Contract. We may include the charges you owe under the Contract on the bills you receive for the Wireless Service. Other terms and conditions for Wireless Service will apply. If you purchase a tablet device, you may terminate your Wireless Service agreement after the first month of Wireless Service without an early termination fee. If you purchase a handset device, you are required to maintain Wireless Service during the term of the Contract.
15-Day Excellence Guarantee: If you are not completely satisfied and decide to discontinue your service within the 15 days, you’ll receive a full refund of the cost of the equipment and pay for only the service you’ve used plus a $35 restocking fee. No early termination fee will apply. Equipment must be returned undamaged and in the original packaging.

Upgrade Program: If you purchase wireless service with an upgrade program, we will accept the device financed under the Retail Installment Contract ("Contract") in good, working condition beginning after the eighteenth (18th) month of the Contract for the unpaid balance of the Amount Financed and you agree to enter a new Contract for a new device. Subject to credit approval.
Plan Limit Notices: As you approach and reach monthly plan limits for voice, text (picture messages not included), data and international roaming usage we will attempt to send you a text message about your usage. Notifications are not sent real time. Usage calculations are conducted nightly and notifications are sent the next day. Plan Limit Notices may not be sent is all circumstances and such notices do not guarantee that you will not exceed your monthly plan limits and incur additional charges for usage. Customer is responsible for all usage incurred that exceeds monthly plan limits. Customers can monitor their usage via My Account. Contact us for more information on other tools you can use to monitor your usage. Service does not guarantee that overages won't be incurred. Not available on certain No Contract Plans.
50% More Data: Get 50% more data for the same price as compared to published Verizon and AT&T shared data plan pricing for current consumer 20GB and less shared plans. Additional data allotment valid for the first 24 months. Standard data allotment applies thereafter.
On-Line $400 Switcher Incentive: Requires purchase of a new Smartphone with Device Protection+ and trade-in of an active Smartphone on former carrier's plan. Limit one $400 reward per ported-in line. Traded-in Smartphone must be in fully functional, working condition without any liquid damage or broken components, including, but not limited to, a cracked display or housing. Smartphone must power on and cannot be pin locked. Upon receipt of new device and account activation, the customer must wait 2-3 business days and then go to the CExchange website to apply for the promotion. $400 Promotional Card will be mailed to the customer after Smartphone is received and condition is verified. Data Plans: U.S. Cellular mobile broadband 3G users can expect typical data speeds of 600 Kbps – 1.2 Mbps for downloads and 400-600 Kbps for uploads. 3G users can experience peak speeds of up to 3.1 Mbps for downloads and 1.8 Mbps for uploads. 4G LTE users can expect typical data speeds of 3 – 6 Mbps for downloads and 1 – 3 Mbps for uploads. 4G LTE users can experience peak speeds of 31 Mbps for downloads and 11 Mbps for uploads. Actual speeds may vary due to area, coverage, compression and the network management requirements of U.S Cellular Customers can expect improved performance and lower latency for many applications running over our 4G LTE network compared to our 3G network, and likewise with 3G network performance compared to our 1X network. U.S. Cellular reserves the right to limit throughput, in its sole discretion, at any time and without further notice. U.S. Cellular also reserves the right to suspend and/or terminate your service, without notice, if you use a data plan in a prohibited manner or if usage adversely impacts (or potentially impacts) our network or service levels. Data plans may be used for: i) intranet access (including access to corporate intranets, e-mail and individual software applications), ii) e-mail access, and iii) Internet browsing. Data plans may not be used for any other purpose including, but not limited to: a) continuous file transfers, b) telemetry applications, c) automatic data feeds, d) Web camera posts or broadcasts, e) automated functions, f) machine-to-machine applications or peer-to-peer file sharing, g) redirecting television signals for viewing on personal computers, or (h) as a substitute for private lines, frame relay or other dedicated connections. U.S. Cellular-approved phone required on all Data Plans. Application and data network usage charges may apply when downloading applications. Not all phones support all applications.
Simple Connect Data Speed: Full applicable data speeds apply for the high speed allotment defined by your plan. Data speeds shall be slowed to 1x thereafter for the remainder of the billing cycle.
Overage Protection: is not available on Simple Connect Plans. Usage tracking is based on included Anytime Minutes and Text Messages, and does not include data usage or Picture Messages. Notifications are not sent real time. Usage calculations are conducted nightly and notifications are sent the next day. Service does not guarantee that overages won't be incurred.
Overage Cap: applies only to voice minutes on eligible plans, and is assessed on a per account basis. Usage is tracked on included anytime voice minutes and does not include data usage or Text, Video or Picture Messages. U.S. Cellular Home Phone: the U.S. Cellular Home Phone Device in conjunction with Home Phone Service is a Commercial Mobile Radio Service and not a landline phone service. 911 calls made using U.S. Cellular Home Phone are routed through U.S. Cellular's automatic location technology and users should be prepared to provide their physical address to emergency responders. Timecard LBS requires subscription to a Data Plan and applicable Timecard LBS plan for each line of service. By establishing and purchasing the Timecard LBS application ("Service"), you consent to having U.S. Cellular provide location based information for each activated handset to Econz Wireless the third party provider ("Vendor") of the Service. Vendor will combine the location based information with the basic account information you already provided to Vendor, thereby allowing for the activation and provision of the Service. When a handset has the Service and is active, U.S. Cellular collects its location information. U.S. Cellular provides that information to Vendor so it will be available to you in your Vendor account. U.S. Cellular and Vendor do not guarantee the accuracy or completeness of the location or other information delivered through the Service, and the Service may be disrupted or fail due to environmental factors and factors affecting satellite usage. A GPS enabled phone is required to use the Service, and the Service is only available within the U.S. Cellular data network. U.S. Cellular does not retain location information longer than necessary to provide the information to Vendor. U.S. Cellular will not disclose your location information to any third party other than Vendor, without additional consent from you. You should carefully review the privacy policies of U.S. Cellular and Vendor, and you should understand the risks involved in disclosing location information. U.S. Cellular and the wireless industry recommend that you inform your end users that location information related to the active handset is being provided to and being used by you. You consent to the sharing of your information with Vendor and to the terms and conditions outlined herein, which supplement and are in addition to the terms and conditions of your Service Agreement with U.S. Cellular and the terms and conditions of the Service, which are available at uscellular.com or any U.S. Cellular Store.
