Device Protection

When protection is needed

You know the feeling. The frantic search through your pockets, your purse, under the seats in your car. You call your number from a friend's phone, hoping to hear it ring... silence. The sinking feeling sets in. Theft, loss or accidental damage—you're covered. Don't waste hundreds of dollars to replace/repair your lost, stolen or accidentally damaged device. Make sure you're enrolled in Device Protection, a plan provided and administered by the Assurant Solutions Companies.

What's covered?

For $5.95 a month, your U.S. Cellular® phone, standard battery and standard charger are covered in case of theft, loss or accidental damage. Device Protection also covers mechanical or electrical breakdown losses after the manufacturer's warranty expires. A maximum limit of $1,500 applies per covered loss.

Wireless Tablets and Wireless Modems.
You can extend the same comprehensive coverage to your wireless tablet and wireless modem for an additional $5.95 a month each.

Easy payments.
You pay $5.95 per month for the Service Contract coverage components (mechanical breakdown and accidental damage). This cost will be added to your account. Your monthly fee will appear on your
U.S. Cellular phone bill. You won't have extra invoices or checks to write.

In addition, you will be the Certificateholder on U.S. Cellular's Insurance Policy for lost and stolen coverage benefits, at no additional cost to you.

The non-refundable deductible for each approved claim for Wireless Tablets is $150. For devices that require a Smartphone Data Plan, Premium Mobile Internet Plan or Data Plus Plan, the non-refundable deductible is $100 for each approved claim. All other models and wireless modems are subject to a $50 non-refundable deductible for each approved claim. Prior to shipping the replacement, the deductible may be paid via credit card (Visa, MasterCard or American Express), electronic check or Cash on Delivery (C.O.D.). A $17 charge will apply for those who choose to pay their deductible by C.O.D.

A free call gets you enrolled.

To enroll, request the plan at the time of purchase or dial 611 from your U.S. Cellular phone—a free call—and ask the U.S. Cellular representative how you can enroll in Device Protection. A U.S. Cellular representative will place a call to your device to validate your service. Your coverage begins when you answer this call.

Making a claim

It's quick and easy.
Simply visit mywirelessclaim.com to file a claim 24 hours a day, 7 days a week, for repair or replacement of your equipment as quickly as possible. You may also call 1-800-480-0167 and a call center representative will work with you to process your claim.

  • Only the person insured on the account may file the claim.

  • Claims need to be filed within 90 days of the incident.

  • Have your wireless number ready so your account can be accessed.

  • You will also need your device's Electronic Serial Number as well as the manufacturer and model/ make/name of your phone. If you do not have this information, you may obtain it by calling
    U.S. Cellular Customer Service at 1-888-944-9400.

  • If your device was stolen, file a police report where the incident occurred and give the police report number to the claims representative. Also call U.S. Cellular Customer Service at 1-888-944-9400 to suspend your service and protect yourself against unauthorized use.

  • If your device is damaged, please provide the cause, location and date of the damage. Damaged devices must be returned to us within 10 days to avoid a replacement and processing fee up to $250.

  • Proof of repair is required when making a subsequent claim on a damaged device.

  • If your replacement device does not work, please notify us and return it immediately using the prepaid and pre-addressed labels provided.

  • In certain events, additional documentation may be required, such as a copy of the police report or proof of loss, to process your claim.

Program Provider Information: Property insurance coverage is underwritten by American Bankers Insurance Company of Florida in all states, except MN, where American Reliable Insurance Company is the Underwriter. The Service Contract Obligor is Federal Warranty Service Corporation in all states, except CA, where it is Sureway, Inc., and OK, where it is Assurant Service Protection, Inc. All are Assurant Solutions companies.

Questions

Be assured that we will make every effort to find the exact model—either new or refurbished—of your original device. If one isn't available, we will replace your device with a model of similar make and quality and will inform you during the claim process if this is the case. Refurbished devices come with a 90-day warranty.

The plan only covers equipment in working order. Device Protection may be terminated or suspended for the nonpayment of your wireless bill.

Terms and Conditions

This is a brief description of the Terms and Conditions and Coverage and Exclusions that apply to Device Protection. A copy of the coverage documents, with complete details regarding benefits, exclusions, and deductible, will be mailed to you when you enroll in this program.

Causes of Loss Not Covered:

A. We will not pay for loss caused by or resulting from:
  1. corrosion or rust
  2. pre-existing condition
  3. failure to follow manufacturer's maintenance recommendations
  4. delay or loss of market, loss of income or interruption of business
  5. intentional and/or dishonest acts
  6. repair or service
  7. power fluctuations
  8. inherent design defects
B. Regardless of the cause of loss, we will not pay the costs which are recoverable under any product warranty.

Term:
Protection starts on the Program start date as shown on the Schedule Page. This is a monthly renewable Program, and must be paid on a monthly basis. If you don't pay the monthly Service Contract charge, Program coverage will terminate.

Cancellation:
You may cancel Device Protection service at any time by notifying us in writing or returning the contract to us. The person paying the premium will receive an unearned premium refund, if applicable.

The wireless service provider's employees are not licensed insurance agents.
Device Protection may duplicate other insurance coverages, such as homeowner's or renter's insurance, which the subscriber may have. Device Protection insurance coverage would be primary to any such other coverage. U.S. Cellular is not qualified or authorized to evaluate the adequacy of the purchaser's existing insurance coverages. Subscribers can check with their licensed agents for a personal insurance assessment.

The subscriber is not required to enroll in Device Protection to purchase or lease the wireless communications equipment. In order to effectively administer Device Protection and to give the administrator the ability to process your claims, you agree that certain information you provided
U.S. Cellular will, in turn, be provided to the administrator. This information will include, but may not be limited to, your name, address, device number, information regarding your device (i.e., make, model and ESN), activation date and information regarding features you have activated.
